- HPA Graduate Planning Sheet (doc): Complete with your academic advisor or with the academic coordinator, De Martin, during first semester.
- HPA Internship Proposal (doc): Must be completed at the beginning of student's internship experience.
- Graduate School Master's Program of Study (doc): Master's programs of study should be submitted on forms provided by the Graduate School at the beginning of the second semester of graduate work. PDF.
- Graduate School Change of Program (doc): Must be completed if your approved Master's Program of Study has changed. PDF.
- Graduate School Committee Change (doc): Must be completed if members of your committee, as listed on your filed Master's Program of Study, change. PDF.
- Graduate School Application for Master's Degree (doc): To apply for degree you must have an approved Master's Program of Study on file with the Graduate School. PDF.
- Graduate School Final Exam Scheduling: To schedule your final
exam, you must have an approved Master's Program of Study and Application
for Master's Degree on file with the Graduate School.

- Graduate School E-mail Release (doc): This form must be submitted before correspondence with a new email address can take place. PDF.
- Graduate School Reenrollment (doc): Must be completed if you have been on graduate leave for more than 12 months and wish to re-enroll at WSU for credit. A $25 re-enrollment fee is also required after 12 months. PDF.
